Campus-Wide Reasonable Accommodation Form
The Accessibility Committee, comprised of representatives from Health Services, Counseling Center, Office of Student Accessibility Services, Housing, and the Office of the Vice Chancellor for Student Affairs, meets monthly and is responsible for reviewing forms submitted for housing and dining accommodations for students with accessibility needs.

Requesting a housing accommodation

- Ensure that all sections of the form are completed thoroughly and attach any additional relevant documentation as needed.
- Documentation from your provider must establish a specific disability that substantially limits one or more major life activities and clearly supports the requested accommodations.
- Committee members and their respective offices cannot advise students or providers on what specific information is needed to secure approval of accommodations. Providers should submit documentation that accurately reflects the student’s condition and supports the requested accommodations.
- Please note that the Accessibility Committee does not contact individual providers directly, nor are Committee members available to receive phone calls from providers. The Committee is comprised of representatives from multiple campus entities and meets on a monthly basis.
- Accordingly, all pertinent information, supporting documentation, and relevant recommendations should be included in the initial submission for review.
- There is no need to schedule a meeting with a department director or submit an amended RAF following a non-approved request in hopes of a different outcome, as individual meetings to discuss, supplement, or reconsider a request are not part of this process.
- Please note: Selecting a specific individual(s) to live with is not considered an accommodation. Students are welcome to choose their own roommate(s), provided they meet the eligibility criteria for residing in the designated residence hall.
*Once your submission is reviewed by the Committee, you will be notified of their decision via your school email.
Submitting an appeal
When an appeal is appropriate
- a change in medical status
- recent hospitalizations
- a new diagnosis (the date of onset must be included)
- a disagreement with the outcome of the original decision
- rewording or resubmitting previously provided information or documentation
How to submit an appeal
Please note: Students have 30 days after the determination email is sent to submit an appeal request.
To submit an appeal, students must:
- email: accommodation.requests@umassd.edu; with the subject line "Appeal"
- Restate original accommodation request and disability
- Attach new or updated medical or clinical information supplied by your provider
- If you feel procedural error was made in the original review, please explain in detail.
Appeal decision
Appeals are reviewed based on ADA standards and the information provided. Students will be notified via email of the final decision.
